If I am not in network with your insurance provider, there are various ways to pay for your visit. Some plans have out-of-network coverage and I can bill them directly. Otherwise, I am happy to provide you with a superbill to submit to your insurance company for reimbursement. If you prefer to schedule online, please include photos of the front and back of your insurance card and policy details in your intake forms.
